Lore of Unseen Lands
Astora is a land that is thematically tied to Knighthood and skilled craftsmanship. Various high quality weapons and gear originate from Astora, as well as several NPCs that include high ranking knights, a blacksmith, and a firekeeper.
It is home to a lineage that believes in the fate of a chosen undead, who must escape the Undead Asylum and ring a Bell of Awakening in order to learn about their ultimate duties. Oscar, a knight of Astora, has a family with a saying that encourages them to fulfill this role:
Thou who art Undead, art chosen.
In thine exodus from the Undead Asylum, maketh pilgrimage to the land of Ancient Lords.
When thou ringeth the Bell of Awakening, the fate of the Undead thou shalt know.
Astora was once assaulted by a dark beast known as the Evil Eye. According to legend, its spirit has been contained in the Ring of the Evil Eye.

Balder was an ancient kingdom that was reduced to ruins after a widespread outbreak of Undead. It was once the home of the legendary Knight King Rendal, who is said to have stood his ground against a giant drake and emerged victorious. Hollowed Balder Knights can be found in the Undead Parish and Sen's Fortress. A comment from the Crestfallen Merchant indicates that Knight King Rendal attempted to conquer Anor Londo and failed. It is likely that his knights followed him and failed as well, left to hollow in Lordran:
Don't even consider visiting Anor Londo. Not in your state.
For a century, they have tried, and failed.
The Knight King Rendal, Black Iron Tarkus, and even Logan himself.
You won't stand a chance.
It is unclear if Berenike is a region, or simply an order of knights, however it is certain that numerous Knights of Berenike journeyed to Lordran after becoming undead. They are known for their heavy armaments and armor. Three hollowed knights can be found in the Undead Parish, Sen's Fortress, and the Painted World of Ariamis, as well as a Darkmoon Soldier in Anor Londo bearing Berenike armor. Their armor set reveals a similar plight to the Balder Knights:
Countless Knights of Berenike, once
extolled as the mightiest of mighty, became
Undead and ventured to Lordran. But their
journey was for naught, as they went Hollow,
and became a threat to all Undead.
The Crestfallen Merchant is equipped with Berenike gear and warns the player about the past failures of those who attempted to storm Anor Londo. His attempt to discourage the player can be interpreted as him being a Berenike Knight who gave up instead of trying to continue forward. He sets up a shop, selling gear looted from corpses, and is on the verge of hollowing just like his fellow knights.
It is unknown if Black Iron Tarkus is a Knight of Berenike, or if he's a lone knight. Though a relationship is considered likely due to his heavy armaments and how he is directly mentioned alongside the Berenike Knights in the Greatsword and Black Iron Greatshield descriptions.
Carim is heavily associated with ominous characters. Terrible rumors surround the creation of the bite rings that were commissioned by Sir Arstor, and the questionable Purging Stone was his secret treasure. The sinister Oswald serves as a Bishop to the goddess Velka, acting as both a pardoner and purveyor of Sin. And Knight Lautrec murders Anastacia the firekeeper, though his motives are unclear.
Velka, the "black-haired witch", is the goddess of Sin. Little is known about her, though she is a described as a rogue deity with strong influence:

influence even as gods are concerned.
Knights of Carim are famous for parrying instead of using shields.

Knights of Catarina wear distinctive, curved armor that often attracts ridicule from others. Very little is known about Catarina, other than being recognized for festivity and drink.
A distant land known for subterfuge, spying, and exquisitely crafted gear. Katanas and slash attacks are known to be one of the main threats in Eastern battles.

A fertile land found at the mouth of a river, home to many farmers. Nothing else is known about the Fivefinger Delta, as it is not referenced outside of the character creation screen.

The Great Swamp
The Great Swamp is the home to many generations of pyromancers, and the origin of various pyromancy spells. Described by Laurentius as being primitive and mixing poorly with advanced culture, pyromancy derives its power from nature. People from the Great Swamp are sometimes considered heretics and face prejudice.
A pyromancer must be in tune with nature herself.

Salaman and Carmina are legendary pyromancers who are also believed to be from the Great Swamp, though their origins are not directly specified. Salaman was a pupil of Quelaana of Izalith who became known as a "Master Pyromancer". Carmina was Salaman's most successful student, who pioneered new kinds of non-fire-specific pyromancies, capable of boosting one's defenses or offenses.
